Let them enjoy a wide range of fun activities at a YMCA Holiday Program, suitable for children aged 5-12 year old.
Balwyn Leisure Centre offer a school holiday program, suitable for children aged between, 5-12 years. We are a regulated, licensed children’s service and follow the National Quality Framework, and Education and Care services National Regulations 2011. Our programs are based on the foundation of working collaboratively with children, educators and forming partnerships with families. We create a welcoming environment where all children and families are respected and actively encouraged to collaborate with educators about program decisions, this ensures that experiences are meaningful. Educators draw on their professional knowledge, observations and discussions to form ideas for each program. We then link this information to the outcomes from My Time Our Place, (which is the curriculum we follow) and choose appropriate experiences to set up the learning environment.
With a variety of exciting excursions and activities, the kids are sure to have a fun, action packed day!!!
The autumn program will be running from Monday 27th June - Friday 8th July 2016 . This program has 50 places available per day. These are allocated on a first in best dressed basis.
Bookings open 9.00am 30th May 2016 and close Wednesday 22nd June 2016 12.00pm
Please note there will be a $5 late booking fee per child, per day, added to your bill if bookings are made after the closing date.
FAMILIES WILL ONLY NEED TO FILL IN A 2016 ENROMENT FORM IF YOUR CHILD HAS NOT ATTENDED IN 2016
ALL FAMILIES WILL NEED TO COMPLETE BOOKING FORM FOR EACH CHILD
We will not be taking phone bookings for this program.
Confirmation will only be given once the relevant forms have been forwarded to us and once you have successfully secured a booking place. Confirmations will be via email and can take up to 5 days.
If emailing your booking may prove to be difficult, please contact the Centre to make other arrangements. All required documents are also available to be picked from the Balwyn Leisure Centre customer service desk.
Please note: Individual Enrolment / Booking forms must be completed for each child. We cannot accept multiple children on the same enrolment / booking form. We still require a booking form for each child. To ensure your child has a position in our Vacation Care Program enrolments will not be accepted without all documentation presented to the Centre. Enrolments sent via email must have all documentation attached.
To book your child into our Holiday Program:
1. Download and complete an enrolment form for each child (NB: a 2016 enrolment form is required regardless of being in a 2015 program)
2. Download and complete a Routine Outing Authorisation Form (only required annually)
3. Download and complete a booking form for each child, indicating which days you would like your child to attend
4. Email the form(s) to email@example.com or submit them in person at the Centre.
5. *Provide other supporting documents to accompany the 2016 Enrolment form which includes; This includes copies of:
- Birth Certificate
- Immunisation Records
- Child Health Record, if available (for sighting only)
- Custody Orders, where applicable
- Medical Management Plans (Anaphylaxis, Asthma or other) where appropriate.
*Families who attended a 2015 program are not required to forward a copy of the Birth Certificate or Immunisation record unless asked to do so by the Centre. If your circumstance has changed, please however ensure you provide updated supporting documents.
If a place is allocated, you will receive a confirmation of booking via mail or email when the booking has been completed, if you do not receive a confirmation within 5 days, please feel free to contact the Centre.
Families may cancel a day of a Holiday Program when:
• Two weeks written notice of cancellation is provided to the service prior to the last day of term; or
• A medical certificate is provided in the event of an absence.
If either of these terms are met a credit to the value of the cancelled days will be placed on the family’s account, less a 20% administration fee.
If less than two weeks written notice is given, prior to the last day of term, a family will only be offered a credit, less a 20% administration fee if:
• The child’s place can be filled by an alternate family.
Refunds will only be processed if the family is not returning to the service.
Lunch Orders will be made available for the winter program. Please note that we have made some changes to the menu and are now providing some healthier options!! All the lunches will be prepared at the Boroondara Sports Complex Café. It is prefered that lunch orders are ordered prior to the program commencing but they can be ordered in the morning of attendence. The Lunch Order form is linked on the right please return it with you booking form.
No credits or refunds will be given for cancelled lunch orders.
It is crucial that all parents are familiar with the policies at the Balwyn Leisure Centre School Holiday Program, and to ensure that all expectations are correct. By signing the back of the booking form you are acknowledging that you have read and understood the YMCA OSHC Family Handbook and understand all policies and procedures including all aspects of the fee policy and agree to adhere to these policies, procedures and payment terms.
Please note, bookings cannot be processed or made without payment and a completed booking form and a 2016 Enrolment Form. ( You only need to complete a 2016 Enroment Form if your child did not attend the January program )
What if Balwyn Leisure Centre is full on the day I would like my child to attend?
Our service works very closely with Ashburton Pool and Recreation Centre, and often the two programs combine for excursions. It is located only 7kms from Balwyn.
